Still need help with the above stated form. Some of the concerns were corrected earlier today but still problems persist.
In the "I am intererested in becoming a Firstaid/CPR Instructor" the last option states "none of the above" When this is selected it should direct the customer to the statement: "Your selection determined that you do not qualify to take this course.... Please contact BreathforLifeInc.....
When the customer selects:
Shipped to Address that you will input on the Next Page (additional Charge ($20.00) the Manual shipping address should appear on the next page. Currently it doesn't appear. I hope this helps.

I checked your form conditions and see that the condition you have set for "none of the above" is to show "Choose cource date" which is working fine.
Please change your conditions and your form should work fine.
I have changed your condition to show the manual shipping address.
Let me reword my request as this does not seem to be understood. I tried to change the condition but it does not recognize it.
If a person selects 'none of the above in the list of choices it should read 'your selection determined that you do not qualify to take this course...please contact..... I tried to change this condition but for some reason it does not accept it. Thanks. Alfred
It works on my end:
Please try to clear your browsers cache. Also clear your forms cache by following this guide: How to Clear your Form Cache
Or, basically you would need to also hide the "Choose course date" field? Let us know!
yes it works but the 'choose course date" is not supposed to appear if none of the above is selected. Thanks for your troubles. Alfred

I've made a fix to your form and added the conditional logic to hide the course date. Please kindly check and let us know for any issues.
The person who changed the form last night did not understand my request for the following.
In the section where it says:
'As there are pre-course assignments for this program'
There are three options:
Option one when selected should show a shipping address in the next page.
Option two and Three should not show the shipping address which is now present in the next page. Are you able to fix that for me please. Option two and three do not allow for a shipping address. Thanks

Is it possible to prevent the date entry's from going to the bottom of the page. Cannot use the calendar as the scrolling bar does not allow the calendar to appear. Thanks.

In the form, check preview, ''I am interested in becoming a first aid instructor..."" the fist option has been chosen. Could you default it so no option has been chosen. Then the form works as it should.
How does one prevent the calendar features from being so low in the form. Alfred
I was able to correct the problem with the first option being selected by default. If you cannot correct the date issue it would be nice to receive a confirmation on that. Thanks
Can you open a new ticket about the calendar problem?
So that we can open a bug ticket about it. Since this thread is about the other problem we cannot assign a bug ticket to it.
As a temporary solution you can enter some spacing below the form using the Free Text (HTML) field under Power Tools on the form builder.
