    Why this happen? Notification said due to phishing? What does it mean?

    As per checked on your account, your forms 9211333901144492231465071451, and 90691171803456 are collecting login credentials which violated our terms of use.

    Online Form Builders, including Jotform, are sometimes being used for identity theft and we go to great lengths to prevent this. That is why collecting these types of data is considered as phishing activity, as stated in our terms of use.

    We will only lift the suspension on your account if you agree with our terms and allow us to delete the offending fields mentioned above.

    1. Form 92113339011444- Paten System Login 
    2. Form 92231465071451- GPIA Albeta Login
    3. Form 90691171803456- MF System Login


    All of these forms are collecting for login credentials that violated our terms of use.

    We will only lift the suspension on your account if you agree with our terms and allow us to delete the offending fields mentioned above.

    Noted, indeed i'm using jotform as a simple login form. A year ago, your
    support team suggested me on not using "password" as field name, so i
    replaced it with "access code".
    Could you provide me with a solution so i can keep using jotform? I only
    need a simple login form by using jotform's conditional feature.

    Unfortunately, we adhere to the rules stated in our Terms of Use and are strict in observing it.

    We do not allow the creation of login forms or anything that may resemble it.

    Could you please elaborate on what you are trying to achieve and maybe we can find better solutions for you?

    Hi, I would need a simple way for my users to access a private page of my
    website. Using jotform, i can do that by having a conditional setting.
    Please advice if there's another way i can do that without violating your
    policy.

    Please note that login forms are not allowed. If you want to password protect your form, please check the following guide:

    How to Password Protect a Form

    Let us know if you agree with our Terms and we will delete the guilty fields from your forms and activate the account.
