Account suspend

    if my account suspended. Can i just remove the company logo Prudential BSN takaful, and use normal email without the business domain

    I checked your form and it is asking for bank account information and credit card information. You can check our terms of use for more information: https://www.jotform.com/terms/

    We need to remove it before we can activate your account. 

    Let us know if that is okay with you so we can proceed to remove the guilty fields.

    But we didnt event ask for the credit card number!!!! check again before making any stupid decision

    Actually, you ask for bank details on your form. This is also not allowed. Please refer to the attachment.

    If you want to reactivate your account, we would need to remove the bank details fields.

    Im not collecting any payment, just bank info for us to make claims later.

    No credit card number was asked, we only ask for bank account which our company will give refund for any claims

    N which widget would help me solve this issue.

    please its a simple info we are asking


    other than the payment anything else?
