Intermittent integration with Mailchimp.

Hi I have one of my forms set up to integrate with mail chimp but the integration is not consistent and sometimes the form details don't make it to mail chimp. Mailchimp checked it Monday and the jot form details were successfully integrated into mail chimp. A lady completed a form yesterday and it still hasn't made it over to mail chimp. Could you take a look please and advise.
