    i was IP ban so I used vpn to post here, and i was forced to use other email since my account is suspended, i was able to communicate in facebook messenger with Jotform, and i also sent an email to as advised, attached all my proof for the account, please give me feedback on this. thanks alot

    I have checked your account and it seems your are suspended due to your form 201694201627451   collecting uploads of ID's and using corporate logos.

    We understand this might not be the case with your form, yet we had to act according to the general rule to be in compliance with our Terms: Free accounts with non-corporate emails showing suspicious forms must be suspended immediately.

    In line with this, we would like to request these information from you:
    1. The URL where your forms are used.
    2. Your website or proof of affiliation to a legitimate institution.
    3. Share with us a document that will prove that you're authorized to use the said logos.

    We also need you to delete the guilty fields in your form and change your registered email to a corporate one to avoid being flagged again by our phishing detector that can lead to another account suspension.

    i already sent an email earlier to, i will resend my explanation and all proofs i can give for reactivation of my account and lifting of IP ban, using the subject you provided,

    regarding ids, since this is an inquiry and lead generation form,F upload files of such ID we are capturing it through upload for verification that the one filling up of the form is not fraudulent, this is why a terms and conditions indicating acceptance of Philippines Data Privacy act in sharing these information are there so that the users filling up of the form is aware of sharing their data.
