I multi pick list with fill in the blank

I would like to see if I could find a solution where I have a multi-picklist with a fill in the blank option as well.

expl:

-trip duration of _____ weeks

-managed crew of ____

-sailed to _____ and to _____ 

I want them to be able to fill in the blanks and be able to choose one or more of the line options

To my understanding, you would like to let your users fill in the blanks and select one of the options at the same time? Unfortunately, it's not possible to combine those two functions on a field or widget.

However, you can check the Passage Test widget to let your users fill in the blanks. May I have the form in question? Can you provide us with your workflow or additional details, to see if it's possible to meet your requirements in the Form Builder?
