My account was suspended

Please unsuspend my account. I am trying to make some forms here!

The reason I had a username and password field on my form is because it is for newly acquired customers to give us access to relevant services.

It seems that your account was auto suspended by the system due to our Terms of Use, the gathering of certain private information such as Driver's License information, Credit Card Details, Login Information, Social Security Information etc is not allowed without mandatory widgets.

You need to include E-Signature widget in your form. However, we also require that you include a Terms and Conditions widget with your terms for your users to read.

I've activated your account as the form I see in your account does not have any offending fields. However, once you have the above mentioned necessary widgets in your form you can use these fields.
