Appraisal form

Hi

Is there a way that a form can be produced emailed to the individual who completes the first part of the form prior to their appraisal meeting and then the manager can fill out the remainder of the form when they meet for their appraisal and then the form can be submitted?

It seems you want to create approval based workflow form. Yes it is possible to achieve your requirement by taking advantage of a combination of different features like "Edit Link", "Condition" and "Get Form Page URL" widget.

BTW, in workflow based form, the form needs to be submitted in multiple stages. Please note that email alert can only be sent after the form is submitted.
