I need Help Getting My Logo on my Document I created

Good Morning,

                 I need help getting this PDF Form completed. I designed the form with my logo it disappeared on the Preview, Then I wanted to complete my form and the by adding more info , then when I tried to sample the PDF it came on 2 sheets. I am having a tough time here. The Original form shows on One Document. I am Confused

Hi guys
I would love to use the second one. If my form can be just like
that in one form.
How was I able to open that from what you sent.
Then when I did mine it comes out completely different.
Can you send me that so I can download it and be able to email it so my
clients can quickly print out.
