the document that we put in there is not visible on the form when you go to sign the form. I haven’t been able to use it but I’ve been wanting to use Jot forms.

I believe you are referring to the embedded PDF Document. The purpose of the widget to just display a document that the user can read. There is no option to sign them there:

If you want you can create a DocuSign account and add you waiver there. Then add Docusign widget to the form and integrate your Docusign account with it. In that way, when the user filled out the DocuSign widget, he'll be redirected to the PDF Document (in your DocuSign account) then digitally sign the document.

DocuSign is a paid service though.

I had it embedded in our site but I took it off because it wasn’t working.
I know the current document is not in a fillable format but I was wondering
if there is a way to make it fillable so I don’t have a separate signature
    line.

Unfortunately, no, there is no way to make the embedded document fillable, but alternatively, you may follow the Docusign workaround suggested by my colleague.

    Thanks.

Ok sounds good but what about my current form. I sent our website to you
all. I embedded it at the bottom. I am not able to see the embedded
document or the calendar. Can you check on that?

I can see you also provided the link to your page where the issue about the embedded PDF on your form does not show on another ticket, we will continue discussing that matter here in order to avoid confusion while posting replies about the same matter on different tickets.

I've now checked your web page and was able to see the issue you mentioned:

Using the direct form URL seems to work:

I would recommend you to kindly try embedding your form using the iFrame code as given by the publish wizard, this guide will help you to get it: https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code

Let us know how it goes.
