Use own Fillable PDF with Jotform

    Also I was trying to get a fillable form to be involved in our sign-up form. Do I have to pay for that feature? If not how do I get a form that we already have in there to where they can sign it.

    Actually, you can import a Fillable PDF Form to Jotform. It creates a web form version that when submitted, the upload PDF will be populated with the data that's added to the webform.

    You'll have an option to attach it to the email that the user will receive when the form is submitted. And the same as the email notification that you'll receive.
