Unable to log in due to suspended account

    I am unable to log in my account via google, nor were I able to reset password. Requested reset but no email recieved.

    Your account was put on suspension due to the following forms and their respective fields that are shown below.

    In order for us to reactivate your account, we need to delete the password fields and credit card fields in those forms listed above. As for the only form asking for bank details, you would need to add not only a signature field but also a Terms&Conditions widget.

    Thanks for writing back to us.

    Please allow us to remove the offending fields from your forms. Only then your account can be reactivated. After adding the widgets you are most welcome to revert the changes.

    Sure, but for the credit card authorization form, since the template is
    available in jotform and such form is also in use in real life, I would
    assume that it can also be fixed by adding some terms or removing some
    fields?

    So we can reactivate your account, we would need to delete the credit card fields in your Card Authorization form. If you need to collect payments from your respondents you can use any of our payment processing apps,

    As you have confirmed to reactivate your account. I have deleted the credit card fields in your Credit Card Authorization Form 信用卡刷卡授权书

    Also, I have removed the password field in the New Customer Registration Form 新客户注册表格

    You may add the Terms and conditions widget to the Bank Authorization (ACH) 银行转账授权书.

    Hi,
    I am adding the widget to my ACH authorization form and I'd like to know if
    there is a general template for the terms & condition? And is the link to
    the T&C only option for it to be added?
    Besides, I downgraded my package, as I do not need that many forms as of
    now, can you confirm the downgrade and will I be getting a refund? I have
    not received any email confirmation for this downgrade yet.

    There is no specific template for displaying terms. You may link to the terms of your web site or display the same. Please see the guide below for adding terms on your forms.

    can you confirm the downgrade and will I be getting a refund?
