How to remove the "Please Run the Wizard" message on my form?

4. The last issue is a strange message that appeared on the front end of the form.  I don't see it while editing the form though.  It reads "Please Run The Wizard".  I'm not sure what this is referring to.  It's on the last tab just before submit.  See screenshot below, and advise how to get rid of it or fix it. 

I was able to see the run wizard message. I don't see any use of the widget in your form, you can delete that from your form, please refer to my screenshot below.
