    I checked and your account was suspended back in February because your form was collecting credit card info with plain text fields. You agreed with our Terms on this support ticket and we deleted the credit card fields and activated your account. However, you added the credit card fields again to the same form. Because of that, your account is now permanently suspended and cannot be re-activated.

    I remember agreeing to the terms and I did not add the credit card field
    back to my form. I do not charge any of my clients for any services. I am a
    travel agent who works with a host company. In order for me to do business
    I have to have clients to fill out our credit card authorization form which
    is the form I submit to my company. The company charges cards, I do not
    charge anything so I do not accept any payments from clients. I just use
    your company so that my clients can fill out the form which is required.
    Clients do not live in my same area and this form that i'm sending out
    cannot be simply filled out over the phone/computer/tablet, nor do most
    people have printers to print it, fill it out and take a picture to send
    back to me. Your company simply suppose let me use your services to let
    clients be able to fill out the form and submit it to me. That's all I am
    doing. I need to be able to access some clients addresses that I have on my
    file because I need to send out viable information, and then I would no
    longer request to use your services.

    Unfortunately, the decision was made by our reviewer's team and it was final. Your account will be permanently suspended.
