    It’s not centered in MOBILE DEVICE browsers, like iPhone Safari, as illustrated in the attached screenshots. No, it is not embedded on my website.

    I checked the form using iPhone 7 and it is displaying properly. Could you please provide a screenshot of the issue so we can check it further?
