Got an email stating my form was no longer active for lack of payment. however, there was money on the credit card and I am not sure why you could not accept payment. Please advise no how to get back up and running.

Your account is already downgraded to our Free Plan. I'd suggest you re-upgrade again by going to My Account - Billing Section. Please make sure to login first before doing so.
Let us know when done so we can check.

A problem like this probably because your bank may have declined the charged taken to your card. I'd suggest you to contact them as well.
