4, The submissions quoted on the pricing screen. Is this monthly or annually. We would either be Silver or gold.

We are just looking into building a form, intergrating to the website. 

The submissions quoted on the Pricing page is monthly. With the Silver plan, you can receive 10,000 submissions per month and with the Gold plan, you can receive 100,000 submissions per month. 

Once you create a form, you can embed it into your website.
