Radio button with textbox

I need a field that presents two radio buttons and a text field associated with the radio button but only one field can be selected and the text entered.  I have provided a visual example of what I need but I cannot get a Jot field to accomplish this.

So for example, if a person selects exempt they can only enter the biweekly salary rate.  Please let me know if this can be accomplished.

We do not have that kind of field but the functionality can be achieved with basic show/hide conditions. Guide: How to Show/Hide a Field

Alternatively, you can use Configurable List Widget but this allows multiple entries on both fields and doesn't support conditions within it.
