    we are unable to login to our jot form account. We are in urgent need to get this back up and running for our business needs.

    Hi Rebeka,

    Happy to help you today.

    Please be informed that, your account has been suspended for requesting License info in your License Details Form. Due to our Terms of Use, the gathering of certain private information such as Driver's License information, Credit Card details, Login Information, Social Security Information, etc. is not allowed without the mandatory widgets.

    You need to include E-Signature widget in your form. However, we also require that you include a Terms and Conditions widget with your terms for your users to read.

    Please allow us to remove the offending please so we can reactivate your account. You are more than Welcome to revert the changes in your form so the fields are restored and kindly add the necessary widgets so you do not get suspended again.
