Responding to different supervisors with forms that have been assigned to different employees?

Here is the senario - I have 1 form that all employees need to fill out every time they are scheduled to work.  If I have 100 employees that are filling out the same form; but they answer to 10 different supervisors - Do I copy the form 10 different times to set up the responses to go to the 10 different supervisors?  All responses do not have to go to all the supervisors ONLY their individual employees that are in their deparments?

I get listing out the employees emails in the INVITE spot under the PUBLISH tab, but cant figure out any other way to send the responses from their respective employees to the individual supervisors.

Hello @MonroeYMCA - You can set up conditions to send their responses to specific supervisors. So what you need to do is to create like an identifier on which employee's response should be sent to a specific supervisor. You can do this by adding a dropdown field to the form (with the supervisor's name in it). The employee will then select the supervisor name.

And based on the selected supervisor's name, you can route the response to that supervisor's email.

Here's an example condition:

So when the user selects JOHN WICK as the supervisor and he submitted the form, the responses will be sent to John Wick's email.
