When I text the link to my form, the link is coming up with the name of the template, not the name of my form. How can I change that? I text the form to a lot of different people, and it is saying it is a parental consent form for some sort of youth ministry instead of my application.

Could you please add an image field in your form to see if the image shows as the thumbnail image when you post the link of the form.

We do not have a feature to directly set the thumbnail image for form share. Depending on a platform where you share a link, the image of a form can be captured or not.

let us know how it goes else we will propose a feature request based on this.

Let us know how it goes.

    Thanks.

I actually have an image on the form, but I'm not worried about the image. The link description seen in the above screenshot shows "Parental Consent and Release for Powerhouse Youth Ministries" (which is the name of the template that I used from your website) when it should say "STAND Application."  How can we get that changed?

    Hi Bmeler,

You can change the name of the form by going to Settings > Form Settings > Scroll down to Page Title.

    I hope this helps.
