Downloading - Missing Components

    We have to download the forms in new document number one version and when we do so the fields do not transfer over fully there are some missing components is there anyway we can fix this I have attached the link to the form. When the user is filling out the form everything is displayed correctly but after we download it from the back and some of the information is missing.

    Since the form is HIPAA-compliant, we can't test downloading submission PDF ourselves due to security measures.

    Can you please check if this issue can be replicated with other forms under your account?

    Also, I've gone ahead and cleared your form caches form here.

    Please give it a try and let us know how it goes.

    Fields are still not transferring to the downloaded version after submission. I cloned the form that didn't work. I also deleted the fields that weren't transferring and re-added them.Still there's an issue. Can, I please get on the phone with somebody to solve this problem? This is ridiculous. This information is crucial  for our staff to know and it's not transferring to our reg forms.     I have cloned your "Clone of FNB Registration Forms" form, and wasn't able to replicate the issue on a newly-created PDF document.

    Please create a new document by clicking on the "New PDF" button as shown below:

    Afterwards, please download the PDF document you created and see if all fields submissions are properly showing up or not.
