I've cloned your form and was able to connect to Square without a problem.
I have also checked your form and it looks like your Square is already connected.
Let us know if this is an issue.

That is the correct form - I had to recreate is using an old for that was already connect to a square account - however it is NOT the correct square account - I tested it and using an old form already connected worked - but when I disconnected and tried to connect to the correct square account - I get the same issue.
This is the same thing that happened to me ALL day today - when I first started with a new form.
