I did check your website, and I was able to find two form URLs in the landing page. I was able to open both the forms without any problem. Please check the screenshot below:

Do you have any other form embedded in your website which is not opening? I would suggest you to please share the direct web page URL where you have embedded your form, and we will take a look.

We will wait for your response.

It appears I may have fixed part of the problem.  However, now the form is not opening on phones.  I had two people contact me about the problem, which I confirmed when I tried on my phone as well.

The form is tournament entry form, website is www.midweekbassaz.com

Hello! I believe that you're referring to your form on this page: https://www.midweekbassaz.com/tourn-entry-form.html

Direct URL of this Form is: https://www.jotform.us/form/70195830832154

This form has no issues when accessed via its direct URL but on the webpage, it is not loading on the mobile device.

I do not see any problem with the form itself. However, it looks like Weebly is modifying the page when running in the Mobile view.

You're using JS embed code on this page. There is another page on your website i.e. https://www.midweekbassaz.com/membership-application.html which is using iFrame embed code and the form shows up in the mobile view.

Can you implement iFrame embed code on this page as well for your form?
You can also try source code embed method in case you face any issues with the iFrame method as well.

Let us know if this solves your problem.
