My weekly website stopped speaking with my jotforms

The tournament entry form on our website, created in jotform, is not connecting from our website.  I tried to re-connect my weebly website to jot form, but it doesn't work.

When I click to connect, it directs to the following address, kenneth.jotform.pro, and I get the following message "file not found"

Is this my problem or yours?

I am not sure what issue is, I am opening your website and I can see the form there and it seems to be working without showing any errors. 

Could you please explain the issue in more detail and provide screenshots if possible?
