my form is not working it states only one entry allowed. how do i fix this?

Hi,
I've checked your form and it seems that you have Unique Submission enabled to check browser's cookies. What happens is, it will only allow a certain user to submit the form once, if they're using the same cookies.
To fix the issue, you will have to disable it or set the Unique Submission to No check.
