Data not tracking to Original PDF.

    On the Kid Zone Cancellation/Change Request Form, two of my date fields will not populate on the PDF.  This is the second form where a date field won't connect even though you can see it linked to the form.  I don't know if it's on glitch on my end or yours, but this also does it on the DOB field on my Background Check form.

    Is there a piece I'm missing?

    Upon checking the addressed form, I have found no submissions associated to it at all as shown below:
