How to do a health screening COVID form for customers to fill out

Need to sign up for a health screening form that our clients can fill out on iPad during entry to salon 

Hello! As you've already created your JotForm account, you should be able to create your forms from scratch or import a form templates from our repository.

2. In case you sish to import one of our form templates, then you can find a long list of available form templates here: https://www.jotform.com/form-templates/category/health

You should be able to find a template for a form that you're looking for in our repository (link provided above) which you can then easily use in your account, make any changes that you wish and then start using.

All our forms can be filled using desktops, laptops, tablets and mobile devices.
