Can you have a fillable form that you send out and all the data it collects tied to a spread sheet?

I am a VP of our Firefighters Assoc.  We just switched to a self funded insurance for our members and it was a nightmare.  Taking hand filled document and trying to put into a spreadsheet.  If so, which plan would you need to have for this?
