How can i reactivate my account ?

    Did my account get suspended because of me not verifying my email?

    Your account is suspended because of this form: 201737706927058. Collecting sensitive information such as Driver's License may violate our terms-of-use if appropriate security practices not followed.

    If your purpose is not to collect Driver's License from the general public, but from your own customers; and if your business is among the list above and you comply with recommendations below, then we can make the exception:

    1.) Add Terms of Use widget (for your users to read and agree). This will surely make your forms not to be considered as guilty of phishing activities

    2.) Add a Signature widget where your users sign and agree to provide such sensitive information.

    3.) Please use your business email address on Jotform profile rather than the public email address you currently use.

    I have now deleted the offending field from your form and activated your account. Please add the mentioned widgets first before adding the driver's license field back.
