Random field showing up and single date/time picker format for mobile.

    Hi guys,


    With this form I've had two issues pop up:

    1) When testing the form, there is a random field that is showing up and it is not in the form builder anywhere for me to delete. "Customer Name", could you please remove this for me?

    2) Do you know how to better fit the Single Date/time picker for mobile use, you can't see the last columns of days in the picker AND can't see the AM/PM of the times on my phone when testing.

    I did check your form #201799106617864, and I am not able to find other fields also like DJ, event, customer, customer phone and customer name. Please check the screenshot below:
