Setup form that has prepopulate content, with validation, and new input

    I have existing data of employee email addresses and item that they owned (some employee would have multiple itmes. 

    I am hoping to create a survey that once employee input their email address, then item list will auto populate only owned by this employee, then employee input item ID. So I can collect the item with according IDs.

    You did not tell what is your existing data source. But if it you can export the data to an Excel or CSV data format, you can use it as the data source for the form using the Spreadsheet to Form widget.

    Using the concept from the user guide, you can have the prepopulate content, validation and new input option on the form.

    Hi Thanks for your help, my source data will be from a spreadsheet, I understand how to import and auto populate the question. How do I auto populate previous answer as case below, user input Email such as test1@agoda.com, auto populate Item name as Apple and pear, for each item i ask the user to put the Item id accordingly, 1234, or 12345

    How do I auto populate previous answer as case below — Are you referring to the previous submissions on Jotform? You have to manually import those into your spreadsheet and import it manually back to this widget https://www.jotform.com/widgets/spreadsheet-to-form.

    Any changes you want to add to Spreadsheet, you need to reupload it so that it will appear when you load the unique id.

    If I misunderstood your question, please clarify.
