How do I pre-populate the data in the form?

Hello, I have followed the process and the Patient ID field populates from the url however none of the other fields do is there something missing from the url ?

Your link is incorrect. That's the reason why it is not working.

First, please try the populate app. Here's a screencast guide on how to use it:

After that, you need to replace the data with the Unique Name. 

Here's the result:

https://form.jotform.com/201814653299966?patientId1={patientId1}&email={email}&name[first]={name}&name[last]={name}
