I would like to be able to Pre populate the empty fields in following sections:

Billing Information- Institutional Billing 

Report Deliver INformation

Specimen Information  

Since you're using the "Multiple Text Fields" widget, you will not be able to populate it using URL parameters.

Please replace it with the correspondent fields as shown below:

Afterwards, you can easily generate URL to populate them following the guide below:
