Why aren't submissions sending to Salesforce?

    For our form Rapid Recovery for a Stronger South Bend - Elkhart Regional Workforce I'm not seeing the data pull into our Salesforce. It is neither creating new Accounts or updating existing. I have mapped all the fields and checked the duplicate settings. Any guidance would be much appreciated.

    Apologies for the inconveniences caused. 

    I have cloned your form and performed some tests, unfortunately, I was unable to replicate the mentioned issue, here are some screenshots about my test:

    I would recommend you to double check your mapped fields, the integration may not work if you've mapped different field types, make sure you have mapped, for example, text box with text box, address with address, email with email fields.

    To test the integration, you may also map only few fields such as name, email and company name. This will help to also determine if the problem is related to the integration itself or due to different field types mapped.
