How to prepopulate URLs?

    I'm trying to make a form for my staff that as they make a choice on the prepoulate form, it will auto populate a link for the cutomer to fill out.  Example is if my cuomer is doing a roku, a security system, and a sonos, I can select those choices on a form and the form will then populate a URL to each of those links that I have built into said form
