Branding of form

What level of service do I need to sign up for to be able to put my logo on the templates? And also to create my own forms?

You can use the Image field to add a logo in the form. There is also a logo option at the top of it. This for the classic form.

Also, Jotform branding will be shown in the form of FREE accounts. Upgrading to any paid subscription has the option to remove it.
