Why my account been suspended?

    Dear Jotform team,

    I would asked question why my account been suspended?

    Is there something wrong. I just used this application to make online form for our office needed.

    Hi Fenti,

    Happy to help you today.

    I would like to inform you that your account was auto suspended due to one of your forms. However, I do not see that form in your account anymore.

    I've reactivated your account to avoid auto suspension of your account please make sure you include E-Signature widget and Terms and Conditions widget in your form if you are collecting private information such as Driver's License information, Credit Card details, Login Information, Social Security Information, etc.
