Why was my account suspended

    hi my account says it has been suspended I use this form in my workplace to enable me to set devices up for customers so that all their accounts are accessible and ready to use when they get home can you please fix this urgently we use this form on a daily basis.

    Your account was autosuspended since your forms "Tech Setup Form 201668367808062" and "YOUR PERSONAL DEVICE SET-UP WALLET 201808355931052" were collecting multiple passwords via multiple fields on the form:

    Collecting passwords is in violation of our Terms which is part of a login credential.

    Therefore I have removed all password fields from your forms and activated your account. Please do not use the term password on the form, else your account will get suspended again.
