If i take gold plan, do we get app for form upload or not

I want to create a form for user to upload their photo (multiple Photo) from their camera only and then we can collect some more information like location (Lat & Long) along with user input details, so will it work for me under gold plan and i also want to know that you will give me a app for that or not & it will work on every phone or not.
