Forgot Password Locked Out

    Hi,

    Forgot password and attempted to reset but did not receive email. Account is now locked.

    User Name: CHLIMED

    Please advise.

    Hello Kristy - Unfortunately, I was not able to find an account with that username. Have you changed it?

    Also, when you reset a password, you need to use the email associated with that account. This is because the email with the reset link can only be sent to the email address associated with it.
