Your form has two email notifications that have each been configured to send submissions to the same email address. That's why you receiving two separate messages each time a submission is made. In order to receive just a single notification, you would need to delete one of them.
1. Load your form in your form builder (My Forms > Click form > Edit > Email Alerts)
2. Click the notification that you wish to delete
3. Click "Delete Email"
4. Save the change
