How to add consent form but still make a survey anonymous?

    Hello, my team and I are putting together a survey for high school students. We want this survey to be anonymous; however, we are still required to gather consent forms from parents/guardians and informed consent from the students themselves. I'm wondering if there's a way to make sure the consent forms go in the same link as the survey, but once they're submitted keep the forms and the survey separate so that my team does not see whose consent form matched which survey? I've seen this done before on other platforms, but I wasn't sure how to do it on JotForm. Thanks!

    Hey there,

    Happy to help you today.

    If I understood you correctly. You would like to include a link to your consent form with your survey form. You can simply add the URL of the consent form in your survey form or in the survey form "Thank You" page.

    If you are not collecting names and emails or any other contact information on the survey form then it won't be possible to see who submitted the form. Even if the consent form has name fields on it, there won't be any way to match it with the survey form.

    Thank you for your quick response! I actually was hoping that that consent form could be the first two pages of the survey so that everyone who fills out the survey would have to sign the consent form first before getting to the survey. So it would all be part of one single link/one big form. We're doing 1,500 surveys so it would be so much easier if it could be done in one sitting, so that we can be 100% sure that each person who completes a survey has also signed a consent form, but the problem is that we can't know which consent form is attached to which survey. Is there any way to do this?

    I've seen it done before when, for example, you're asked to complete an anonymous survey but then they let you input your information at the end for a drawing (as a thank you for completing the survey), I just don't know how to do it myself.

    Hi there,

    Thanks for writing back to us.

    You can insert a consent section to your survey form. You can look at these templates to get an idea about consent forms: https://www.jotform.com/form-templates/category/consent-forms

    If you don't want people to fill the survey without filling the consent form, you can hide the survey questions for them by using "Conditions". You can take a look at this simple user guide: https://www.jotform.com/help/316-How-to-Show-or-Hide-Fields-Base-on-User-s-Answer

    Another option is that you can use one of the "Short Scrollable Terms" and "Terms & Conditions" widgets. Still, you may need to use conditionals if you want to hide survey questions, etc. 

    For the remaining part of your question that the consent information should be hidden from you as well, achieving this in one form may not be possible. But if you use two forms then you can easily achieve it.

    Create a consent form and provide a link to your users only for the consent form. Now add the link to the survey form in the consent form which will become visible only after the consent form has been submitted. Now when they submit the survey form you won't be able to find who actually filled the survey form.

    Just want to add one more thing. Instead of showing a survey form link on the thank you page, you can also redirect to an external link (in this case survey form). For example, if the consent form submit button text is changed to "I Agree" the next page that will open will be the first page of the survey form.
