Suspended - Unable to open my account

    Hi, I was creating a form when I got a message saying the form was disabled, now I'm not able to log back into my account.  I sign in with Google, but it just redirects me back to the login page.

    Your account got suspended by our anti-phishing tool. The form is collecting credit card information which is against our terms of use. 

    We need to remove the guilty fields before we can activate your account.

    Hi, yes I was not aware it was against the terms of use.  I noticed there are several templates for credit cards and was attempting to make my own.  Please activate my account, thank you

    I have checked your account further and found out your account was suspended not only because of the forms in your account's Trash folder that have the following fields that need to be deleted.

    Your account also has the following forms that have the following fields that collect sensitive information from respondents.

    Those fields require that you have in your form the Signature And Terms&Conditions widget that will serve as proof that your respondents agreed to have their sensitive information collected.

    Another issue that your account has is that the forms are being used to offer loans.

    I have deleted the credit card fields on the forms above. However, in order for us to reactivate your account, we will have to delete the offending form.

    Or, kindly provide us the following information:

    1. The URL where your form is used.

    2. Your website or proof of affiliation to a legitimate institution.

    We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.
