I want to create a client intake form for our non-profit

    Hi,

    I want to create a client intake form for our non-profit. We will have it filled in off line when they come to the office probably on a laptop. Sometimes if we go out of the office we will want to email it back to the office.

    Can you assist us in the creation of this form? We have all of the list of information and questions, which will be yes/no answers. What is that cost?

    What would be the monthly cost for this one form?
