We are not receiving notification of form submissions. The email in the for is correct. It is firstname.lastname@example.org.

I have edited your question to remove the password. We don't need your password to examine your account. However your password would have been visible in this thread for several minutes so you should consider changing it now. I will investigate your problem and get back to you shortly.

Did you ever get any mail from this form or did it just stop working for you?
I have created a clone of your form in my account and set the address to my own, then submitted it and successfully received the notification. So it looks like there is a problem either with your mail box or your mail server. Can you check your spam folder to see if it ended up there? Failing that you could notify the person that administer your email server and ask them to whitelist the following IPs
