hi guys, we are receiving form confimraion email many times every time any of our patients submit the medical questionnaire either both Italian and English version.
Can you please check why are we receiving 4-5 email each time?
it is also connected with slack and we receive same amounts of confirmation there too.

I've checked your form and there actually are duplicated submissions. However, I was not able to replicate your issue on the cloned form. Could you please remove existing email notification and set up a new notification instead?
Steps to remove existing email notification:
Once you remove that, you can set up new email notification by following instructions given in this guide below:

Please give a try and then check your form with test submission if the issue still persists.
