send a pre-filled form

I am coming back to try this out again after many months away.  What we are looking for is a way to send a form  like Docu-sign to third parties and have them sign out some fields in a form, and then have the form sent to employees for them to fill out the rest.  Also we are looking for a way to send one time forms to doctors (all under HIPAA) so they can quickly sign a form or two on their mobile.

The form itself should have the ability to auto fill commonly used/repeated information such as name and even signature.  It would allow some if/then logic to open some fields and close others based on selections.

Along with the ability to send out forms, we would also like the ability for our staff to pull up the form on their ipads to fill them form out in person, if the patient is on site.

Can you let me know if the above features are included in your product?

You can have parts of your form pre-filled by using our Prepopulate App. This will help you generate URL parameters that will fill in values to your form after the URL is clicked.

The mentioned app can auto-fill most of the regular fields. However, that does not include the signature field.

Along with the ability to send out forms, we would also like the ability for our staff to pull up the form on their ipads to fill them form out in person, if the patient is on site.

The staff can bookmark the form's direct URL on their iPads so they can access the form easily.

Alternatively, you can install the JotForm mobile app on their iPad so they can fill out the forms anywhere even without an internet connection.
