When ig go to http://www.foxmls.com a modal window opens with a form inside (http://www.foxmls.com/Flash-Page-Choice-15-minute-Listing.html).
Probably one ot of ten ten times the script does not run and all the fields are showing as can be seen in attachment.Page URL:

It's most probably due to script conflict. Note that you have another jQuery script runs on the page where the form is embedded. It might conflict with the prototype script used in the form, this is why the condition is failing at some point. I was able to replicate the problem but it's happening very intermittent. However, most of the time it works fine on my end.
Try adding this noconflict script in your page after the LMStoreWindow.js script, this might help. It's worth a try.

Please let us know where did you see this error (<script> tag must have type attribute)? Are you getting this error in your browser?

Do you prefer using iframe embed code? This is the best solution to avoid conflicts since the form loads in an iframe. This should fixed it. The downside here is that you have to define a fixed height for your iframe since it's not like the default script embed code where the length of the form automatically adjust everytime a new element is shown.

I see, I got it.
It seems pretty hard to track which particular script conflicts to the form. What we can do here is to do trial and error. Can you please try to replace your jquery.js file to this jquery (this is an original jquery used in your form), this might help.
If this doesn't make any difference, try to remove the jquery library declared in the form since it would be redundant having two jqueries called in the same page.
Make sure to clear your browsers cache when making a test. If none of these works, let us know here. Thanks!

Also, have you tried using only the source codes for this page: http://www.foxmls.com/Flash-Page-Choice-15-minute-Listing.html ? Remove extra codes from the page. Just the plain form source codes.

Oops! I didn't notice that in Firefox. I'm just viewing the page directly with IE. Yes, use the form source codes. You can get it here: http://pastebin.com/0wndb9uC or by following this guide: How to Get Form Source Codes.
Your page: http://www.foxmls.com/Flash-Page-Choice-15-minute-Listing.html contains additional codes and that may caused the issue. Using just the form source codes may help you resolved this. Please try that and let us know for the results.

I chnaged my strategy. I know go to a non form page first. The only issue is in firefox. Go to:
chose Illinois as the state, then press F5 to refresh and start over. It does not clear. It works in Opera, IE and Chrome.

I tried your website page http://www.foxmls.com/Flash-Page-Choice-15-minute-Listing.html using my Firefox browser and I could not seems to reproduce the issue anymore.
I was able to refresh and reload the page to its initial state similar to using its direct URL http://www.jotform.us/form/31966802170151
Since the form source code was already hosted in your website, it seems to me you need to clear also the cache of your website to clear the old web page from being loaded.
when I initially open this page URL http://www.foxmls.com/Flash-Page-Choice-15-minute-Listing.html , it shows the form with already a filled in info. This might be a remnant of a cache on the website. But when you reload the page, it will now restore to the normal state as the form.
Hope this help. Please inform us if the issue persist.
