Our form called HIPAA doesn't have the option to display appointment location in the subheader of the inbox.

We checked the show/hide fields to be sure the patient name and appointment location are showing.

Unfortunately, not all fields are supported to be used as Headers/subheaders. Please refer below to the list of fields that it supports:
Name Field
Short Text Entry
Date Field
Phone Number
As checked, the Appointment Location field is a widget.

Thank you for your message. We have widgets on other forms and the field, appointment date is able to be displayed in the Inbox. So I do believe it is possible for a widget to be displayed in the Inbox, but it isn't working properly on our form titled, HIPAA. Please let me know. Thank you for taking a look! Emily Beglin

Here is one with the appointment location widget! It works on this form. Displays appointment location in the Inbox.

The fields type are not the same on both forms. On the Tyler Borst-... Form you are using a Dropdown element, and on the HIPAA you are using the Dropdown with paging widget.
Unfortunately, not all fields are supported in the header like John said above. I recommend you replace the widget by a Dropdown element if you wish to add the location to the header. Understand that deleting a field from the form will delete the associated data.
